What you are talking about is a REALTIME protection which clam in its native form doesnt do.  Also such protections are done at mail SERVER stage or at operating system level.  As thunderbird is neither, you either have to have it on the client DEVICE as a realtime protection (as an AV product installed) or rely on it being implemented at your mail server stage (if you are in charge of such things).

If you are a windows user, look up 'clam Sentinel' -  this is a REALTIME system based protection based on the Clam project.  (I dont know anything about Linux AV realtime protection software).
